The primary challenge in musculoskeletal (MSK) care isn't the surgery; it's the systemic friction. Before CareNexus, patients navigated a disjointed landscape where referrals were manual, data was siloed, and recovery was delayed by administrative lag. We saw thousands of injured workers falling into the "hidden cracks" of a fragmented system. Traditional models failed to scale because they relied on human-intensive coordination for every single decision point. We needed to automate the clinical logic used in the OR and the clinic to ensure every patient reached the right provider at the right time.

Project execution.

We approached this through the lens of "Surgery to System Architecture." We treated the care pathway like a surgical procedure—every step must be precise, standardized, and measurable. By architecting CareNexus as a cognitive middleware platform, we codified clinical logic into an autonomous reasoning system.

The framework utilizes Triage Logic to instantly assess injury severity and Persistent State tracking to monitor patient progress in real-time across a distributed network of 16 regional hubs and 150+ providers. We integrated this with the CareNexus Intelligence Platform, allowing for real-time KPI tracking and automated referral management. This removed the burden of manual coordination, allowing the system to function as a high-performance engine for functional restoration.

Project results.

The implementation transformed Ontario’s approach to workplace injury and low back pain. By orchestrating a clinical network of over 10,500 registered primary care providers, we removed the friction points that lead to chronicity.

The results are quantifiable: 37,000 injured workers served with a 93% patient satisfaction rating. In our Rapid Access Clinics for Low Back Pain (RAC-LBP), the automated triage system generates over $3M in annual savings by reducing unnecessary imaging and specialist consultations. We didn't just build software; we architected a system that restores human capability at a provincial scale.
